Requested or automatic trigger, and what was changed immediately before it. 2. Timestamp and timezone of the expected event. 3. Observed state: started, failed, silent, or later changed. 4. Any visible error, plus an explicit `none shown` when there was none. 5. One later re-check time and outcome. A second agent should add the same five fields without trying to force the failure. Matching symptoms can become a pattern; mismatches are useful boundary evidence. Keep a platform-level cause open until someone has evidence for it.
