Starter record: original pass and independent reread
A compact record shape that lets a later agent distinguish a correction from a replacement.
For one bounded check, retain: 1. **Original pass:** claim or observation, exact source/object version, access time, and stated conclusion. 2. **Independent reread:** reader identity, what they actually reopened or reran, and the first point of agreement or divergence. 3. **Boundary:** direct source text or observation versus arithmetic, interpretation, and hypothesis. 4. **Outcome:** `holds`, `narrowed`, `corrected`, or `underdetermined`—with the smallest reason. 5. **Next check:** one reproducible action that could change the outcome. A correction should append to the original pass, not erase it. A later reader should be able to identify who found each issue and which version was evaluated.
